NEW PATIENT PACKET
Click on the link in the sidebar (left) to download our “HCA New Patient Packet.pdf” with new patient intake forms, and a map. Please bring completed forms with you.
(If you have trouble downloading/printing the packet, we also have paperwork at the clinic. Please come to your first appointment 15 minutes early.)
SOME TIPS FOR NEW PATIENTS – WHAT TO EXPECT:
Schedule your first visit: Use our handy-dandy online “BookNow” scheduling system, or call us during clinic hours if you have questions and want us to help you schedule an appointment.
Download and review New Patient Packet: Before your first visit, you may want to look over our New Patient Packet. You can print and complete your paperwork now (see download button in lefthand sidebar), then bring it with you to your first appointment. Please arrive 15 minutes early.
Allow 90 minutes for your first visit: Plan to be at the clinic for about 90 minutes. This allows enough time to complete and process your paperwork, and thoroughly review your health issues with your acupuncturist. Your first visit also includes a treatment. Follow-up treatments for returning patients are generally 60 minutes.
Arriving on time: We realize your time is valuable and we want you to get the most out of each visit. Therefore, we ask that you make every effort to arrive on time for your appointments. If you get here late and other patients are waiting, we may need to reschedule you.
Payment: We ask that you pay when you sign in at the front desk. In addition to the $15 – $40 for your treatment, there is a $15 Initial Intake fee on your first visit only.
Insurance billing: We do not process insurance claims. However, upon request, we will gladly issue a receipt that you can submit to your insurer.
What to wear: Wear loose, comfortable clothing with sleeves that can be rolled up to the elbows and pants that can be rolled up to the knees. You’ll need to remove your shoes and stockings, but nothing else.
Fragrance Free Zone: Many of our patients are sensitive to smells. Please avoid wearing any perfume, cologne, or scented lotions to your appointment.
Cell phones and other electronics: Out of consideration for others, and to get the most out of your own treatment, please completely turn OFF cell phones and ALL other electronic devices before entering the treatment room. We will have soft, soothing music playing. You may bring your own music (with earbuds) if no one else can hear it.
Eat something beforehand: It’s a good idea to not get acupuncture on an empty stomach, so please eat a little something ahead of your appointment.
Non-urgent scheduling & cancellations: To schedule or cancel an appointment one or more business days in advance, you can use our online schedule system, or call us during clinic hours.
Same-day cancellations & appointments: Please be aware that we require at least 24-hour’s notice for cancellations, so we have time to re-fill your spot. We may charge a $15 fee for appointments that are cancelled with less than a 24-hour notice. Please email or call us at 503-282-9777 as soon as possible, so we can offer your slot to someone else.
A word about parking: Parking can be a challenge at peak hours. Please allow extra time to get to your appointment if you are driving. Remember, parking in the Trader Joe’s lot carries huge financial risk. You could probably get a lifetime supply of acupuncture treatments for the cost of one towing charge…read more about parking.
Above all—enjoy yourself! If you have never had acupuncture before, you may be a bit nervous at first, but you’re in for a real treat. Your acupuncturist will review your health history with you, listen to your health concerns and goals, carefully explain the process, and will help ensure that you are relaxed and comfortable at all times. Never hesitate to ask questions. You are in a safe and healing space.
Visit the FAQ page for more answers to questions you never even thought about asking. Or call us during clinic hours.
We look forward to meeting you!
